ご提供いただいた情報は、ポートランド日本人補習校（PJS）が学校運営上必要な目的に使用し、適切な方法で管理します。また、システムの運用・保守等のため、PJSが委託する事業者が必要な範囲で情報を取り扱う場合があります。
PJSでは、個人情報への不正アクセス、紛失、漏洩等を防止するため、合理的な安全管理に努めています。また、収集する情報のうち、メールアドレス、住所、電話番号、生年月日などのセンシティブな情報については、万一データベースへの不正アクセスが発生した場合のリスクを軽減するため、暗号化した状態でデータベースに保存しています。
しかし、このような対策を講じた場合でも、インターネットを利用した情報の送信・保存に伴うリスクを完全に排除することはできません。第三者による不正アクセス、システムやサービスの障害、その他予期しない事象によって、情報が漏洩、紛失または不正に取得される可能性があります。

This survey collects personal information about students and their families to the extent necessary for school administration and to provide appropriate education and support to students.
The information you provide will be used by Portland Japanese School (PJS) for legitimate school-related purposes and will be managed with appropriate safeguards. Service providers engaged by PJS may also access or process the information when reasonably necessary for system operation, maintenance, or support.
PJS takes reasonable measures to protect personal information against unauthorized access, loss, or disclosure. Sensitive information such as email addresses, addresses, telephone numbers, and dates of birth is encrypted when stored in the database to reduce the risk associated with unauthorized access to the database.
However, even with these safeguards, the risks associated with transmitting and storing information through Internet-based systems cannot be completely eliminated. Unauthorized access, system or service failures, or other unforeseen events could result in information being disclosed, lost, or accessed by unauthorized parties.
